Our Services

At The Care Concierge, we can help you plan for and provide care for your loved one. Just like our name says, we are your concierge! Anything that you need, we will be happy to provide for you.

Because everyone's needs are different, we offer two distinct packages.

Placement Package

Our Placement Package helps you plan for every aspect of transitioning into a different type of care need. We realize that change can be difficult, which is why we are helping you through every step of the process. And if you need help with something that isn't mentioned, all you have to do is ask.

Just some of the areas we can assist you with are:

  • Non-medical needs assessment to help you understand ADLs (activities of daily living) and IADLs (instrumental activities of daily living) and how they can determine the type of care required
  • Research and recommendations on all options for home-based care and facility placement based on family needs and wishes

  • Accompaniment to recommended facilities for personal review and assistance
  • Accompaniment to visit with home-based care providers for review and assistance
  • Coordination of moving and transportation services
  • Access to all of our best Chicago partnerships

Guidance Services

Our Guidance Services are designed to help you to prepare for the legal, financial and planning needs with long-term care. We have partnerships with the some of the best providers in Chicago, and we will share all of our information and knowledge with you to ensure a smooth transition.

Some of the aspects we can help you understand and plan for are:

  • What is and is not covered by Medicare and how you might be able to use Medicare to your advantage
  • What types of long-term care insurance policies are available and how they can be used to your benefit
  • What advanced directives are and how you plan for and administrate them
  • What the different types of power of attorney are and for what you need each
  • How to discuss end of life care and family wishes with those you love

Here are some examples of folks and situations and how we can help:

George & Michelle

George still lives at home, but is struggling with the recent loss of his wife. He is not very will versed in how to keep house or do laundry. His daughter-in-law Michelle is visiting him once a week to take him shopping and clean the house. Michelle would like for George to have someone come and clean and help with laundry, as well as help George get to doctor appointments and the occasional shopping trip.

How we can help

We would work closely with Michelle in order to ensure the best possible care for George. Depending on what Michelle thinks is most important, we can provide many resources. For example, we give Michelle a list of support groups that might be able to help George cope with the grief that he is feeling. We also provide Michelle with a list of recommendations for some local housekeeping companies that could come and make sure that the house is cleaned weekly. We also provide Michelle with recommendations for non-medical home healthcare providers. These home healthcare providers come visit George to help with cooking, cleaning, laundry, and to give him some company during the day. After we get George set up, we make sure to keep in touch and make sure that he's doing well.

Edwina & Todd

Edwina has had perfect health up until now. She has recently had a stroke and has been hospitalized. Todd is her son, and is unsure what to do in order to provide the best care for his mother after she is released.

How we can help

This is a hard situation for anyone. Because of Edwina's stroke, she will probably be discharged into a skilled nursing facility to receive various types of therapies. After Edwina is feeling much better, she and Todd have decided that it's best if she moves into an independent care facility and not back into her own home. We can assist Edwina and Todd here in many ways. First, we help Edwina and Todd to understand what types of facilities are available and what types of services they provide, and provide recommendations of facilities that best fit their needs. We provide some recommendations to Edwina and Todd for some movers to help transition Edwina to the new facility. We help Edwina and Todd through the advanced directives and make sure that Edwina and Todd are set up with some recommendations for financial planners to ensure that Edwina can pay for her care and still have something to leave for her family. Finally, we make sure that Todd is set up with appropriate power of attorney paperwork so that if Edwina has another stroke that Todd has the legal capacity to take care of her.

Alice & Richard

Alice and Richard are an older couple here in Chicago and getting to the point where they can't maintain their large house as easily anymore. Alice and Richard would like to downsize into an independent living facility.

How we can help

First, we provide Alice and Richard with an overview of different types of facilities in the area and then we accompany Alice and Richard to visit the facilities. We recommend some moving companies for Alice and Richard. We also keep in touch with Alice and Richard to make sure that the facility is meeting their needs and help them arrange any extra service they might need.

Clark & Helen

Clark and Helen's kids have just left home and are planning for their retirement. They want to know what they can expect as far as costs and facilities available for long-term care. They also are unsure what long-term care insurance is, but think that it might be worth looking in to.

How we can help

We help Clark and Helen evaluate the options available for long-term care. We help them to understand what is involved with purchasing long-term care and what they can expect from some of the benefits. We can even recommend some long-term care insurance providers in their area. We help them get their advanced directives in place as well as power of attorney for their children. We can even recommend some quality financial planners to help make sure that their assets and funds are protected and maximized to ensure a steady stream of income after retirement. We will work with them every step of the way to make sure that they are all set!

Maria & Tony

Maria has a good friend, Tony, with whom she has been good friends with for many years. Maria has no family of her own and no one to help take care of her. She trusts Tony and would like for Tony to play a much bigger role in her care in the future. Tony wants to help as much as possible, but because they are of no relation, he is unsure how to help.

How we can help

Maria's situation is becoming more common. To help Maria, we recommend some very skilled attorney's who can help Maria draft a power of attorney so that Tony can make medical and financial decisions if Maria needs him to. We also discuss her wishes for advanced life directives and make sure that Maria describes her wishes in writing. We also make sure that Maria and Tony have fully discussed all of her wishes –- medical, financial, etc. -– so that if need be, Tony is fully prepared to help.
